  • We will have to disclose protected health information to another clinician/physician who may be treating you when we have the necessary permission from you to disclose your protected health information.

  • Healthcare Operations. We may use or disclose, as needed, your protected health information in order to support the business activities of our practice. These activities include, but are not limited to, quality assessment activities, employee review activities

  • Health Oversight. We may disclose protected health information to a health oversight agency for the activities authorized by law, such as audits, investigations, and inspections.

  • You have the right to inspect and copy your protected health information. This means that you may inspect and obtain a copy of protected health information about you that is contained in a designated record. Please contact our Privacy Officer if you have questions about access to your medical record.

 

Complaints:

 

  • If you believe your privacy rights have been violated, you may file a complaint by speaking with or writing our Medical Director at Irie Height’s LLC. The Medical Director can be contacted (571) 330-6328. You may also file a complaint with the United States Department of Health and Human Service.
